The link to the instructions are dead but here is what you do:
From the DL site click add to cart (large red button with shopping cart). Then from the shopping cart page you are going to click on the brown button (has a card on it, top left and right of the big red button for members).
In the first field fill out your name, in the next field your credit card number, exp date, security code, then phone number, email, confirm email, mark the first box which starts DLsite, then click the larger button on the left to submit.
I've bought some of Kisirian's work in the past and posted here, unfortunately my new credit card company does not allow purchases from dlsite.
945 yen is about 10 dollars.
